الجراثيم الممرضة المرتبطة بجروح الكسور المفتوحة: العزل والتحليل الجرثومي الدقيق

الملخص

تم في هذه الدراسة جمع 145 عينة من اصابات جروح الكسور المفتوحة من المرضى المصابين بالكسور، المراجعين للعيادات الاستشارية وردهات جراحة العظام والكسور لمستشفى الجمهوري التعليمي ومستشفى السلام في مدينة الموصل زرعت العينات على الاوساط الزرعية الخاصة وشخصت العزلات اعتمادا على الصفات الشكلية والزرعية والاختبارات الكيموحيوية واختبار API20E  (Analytical Profile Index 20 Enterobacteriaceae system) اختبرت حساسية العزلات الجرثومية للمضادات الحيوية باستخدام طريقة الانتشار بالأقراص. تم الحصول على (137) عزلة من الجراثيم الموجبة والسالبة لصبغة كرام وبنسبة 94.5% من مجموع العينات، كانت جرثومة S. aureus، أكثر الجراثيم الموجبة لصبغة كرام عزلا وبنسبة 50.9%، فيما كانت جرثومة P. aeruogenosa  أكثر الجراثيم السالبة لصبغة كرام عزلا وبنسبة 30.8%، في حين تباينت نسب عزل الانواع الجرثومية الاخرى. أظهرت الدراسة ان نسبة حدوث الاصابات في الذكور اعلى منها في الاناث ، حيث بلغت في الذكور (76%) فيما بلغت في الاناث (24.1%)  ، وفي داخل المدينة اعلى منها في خارجها حيث بلغت في داخل المدينة (77%) فيما بلغت في خارجها (23%) ،كما كانت الاصابات في الاطراف السفلى اعلى نسبة من الاصابات في الاطراف العليا حيث بلغت الاصابة في الأطراف السفلى)( (69.3%)  بينما في الاطراف العليا  (30.7%) ،وقسمت العينات حسب العمر الى ست فئات عمرية وكانت الفئة (18-34) سنة الاكثر عرضة للإصابة وبنسبة (60%) .كما قسمت العينات حسب تصنيف Gustilo and Anderson  الى ثلاث انماط وكان النمط الثاني هو الاكثر نسبة بين العينات (35.2%). أظهرت نتائج الحساسية للمضادات الميكروبية فعالية مطلقة لمضاد Imipenem ضد العزلات الجرثومية السالبة لصبغة كرام، في حين كانت هذه العزلات مقاومة بنسب عالية لمضادات Amoxicillin-clavulinic acid، Rifampin، Amoxicillin، Carbencilln. أما الجراثيم الموجبة لصبغة كرام فكانت حساسة بنسبة 100% لمضاد الـ Vancomycin وعالية الحساسية لمضاد Rifampin وعالية المقاومة لمضاد Cefipime.
